ANSWER
Air crescent sign.As there was no significant response to the Ambisone it was augmented with Caspofungin. On July 25th there was clinical improvement, she was afebrile, and had a normal WBC count. She continued to improve till Aug 21st when she suffered a massive hemoptysis of > 400ml one night.
